My 2001 Subaru Outback with 200K+ miles starts to sporadically pull very hard to the right after driving about 90 miles.

When the engine cools, the pulling stops.  When the car pulls, the wheel turns too.  It gives about 3 or 4 strong pulls, then drives fine for a while, then pulls again.  What is causing this?

Sounds to me like it's either the P/S pump or even the steering rack pulling the steering wheel. It is a possibility of a separated tire, but it's doubtful. 

I would lean more towards the power steering pump surging and pulling the wheel. Have this checked immediately though, as it is very dangerous.
