2006 ford fusion transmission problem?

Transmission slips when accelerating from 4th to 5th gear, and the engine revs (high rpm's) for a minute before the transmission re-engages. Also, the transmission does a hard down shift from 2nd to 1st when coming to a stop. What is the cause of this problem?
